How to Enroll in Services
Step 1: Contact a case management entity (LCDDS or Brokerage)
First, the person needing support services must be assessed for service eligibility by contacting Lane County Developmental Disability Services (LCDDS) or a qualifying local brokerage. Whichever case management entity you choose to pursue an assessment with will help you connect with your preferred services through a referral process.
Step 2: Request a REfferal
After the person seeking services has selected a case management entity, request a service referral package be sent to your provider of choosing.
Step 3: Meet with our Associate Director

Every referral is carefully reviewed by our Associate Director to ensure we have the capacity to provide high-quality, individualized services. If we’re confident we can meet the person’s needs and foster a positive experience, we’ll arrange a warm meet-and-greet to start building that connection.
